Pediatric Prescriptions

For all medication refills, please call the office or request via our e-mail form. Please have the following information ready: the patient’s name, date of birth, and phone number, the name and phone number of the pharmacy.

Medication prescription refills may be disbursed to a patient/parent/guardian if requested with at least 2 business days notice. Whenever possible, prescriptions will be called into a patient’s pharmacy. For controlled substances, prescriptions can either be mailed to the patient’s home (see below), or picked up at the office during regular business hours. Should an individual other than the patient’s parent pick up such a prescription, this information MUST be shared with the office staff PRIOR to their arrival. Valid identification must be produced prior to leaving the office with the prescription. New patients not yet seen or evaluated by Pediatric Wizards who request a medication refill may need to make an appointment prior to a prescription being given.

Prescriptions By Mail

Beginning on January 1, 2010 Pediatric Wizards will offer our patients an option in order to receive mailings from our office.

You can provide our office with self-addressed stamped envelopes for required mailings. If you require monthly prescriptions, we can keep several self-stamped envelopes in your child's chart for your convenience.
